Caregiver Reference, Identity and Work-Eligibility Checklist

A consent-based checklist for verifying information relevant to a direct caregiver hire.

·

Ask permission and explain purpose

Tell the candidate what will be checked, why it is relevant and who will receive the result. Collect no more personal information than the check requires.

  • Written or recorded consent
  • Identity matched to the candidate
  • Work eligibility reviewed
  • Credential issuer contacted where relevant
  • Check records stored securely

Contact references independently

Use contact details you can reasonably validate and ask about dates, duties, reliability and eligibility for rehire. A reference is one input, not a guarantee of future performance.

Treat background checks carefully

Check type, consent, relevance, privacy and human-rights obligations vary. Do not promise that every candidate has passed a check unless that exact check was completed and its result was assessed for the role.
